Health Forms
Forms for immunization exemptions and for medication given during the school day. Print, complete, and return the signed form to the office.
- Permission for Medication Administration Required before school staff can give a student any medication, prescription or over the counter. Needs a parent or guardian signature and a health care provider authorization.
- Certificate of Medical Exemption Colorado immunization exemption on medical grounds. Submitted once, unless the student’s information or school changes.
- Certificate of Nonmedical Exemption Colorado immunization exemption on religious or personal grounds. Must be filed each year during enrollment.
The exemption certificates are Colorado state forms. The current versions are always available from the Colorado Department of Public Health & Environment.
